Domain 1- Knowledge of the Nursing Practice 


My Definition - Knowledge of the nursing practice is to not only have proficiency in the skills of generalized nursing but also the role the nurse plays on a multidisciplinary team, the nursing theories that are fundamental to nursing practice, and the history of nursing. Knowledge of the nursing practice would only be attainable with the prerequisite coursework to enroll in a nursing program. All coursework, including algebra, chemistry, biology, and English composition and psychology, are fundamental coursework built upon in the ASU nursing program to give a student the tools to build knowledge and evidence-based and safe nursing practice.


Where I learned It -My prerequisites in chemistry have been fundamental to my success in understanding the nursing process. Organic chemistry allowed me to think in what I perceived to be a different language.  How molecules and specific atoms interact helped me understand why and what kind of chemical reactions were taking place. Organic Chemistry was complicated for me at first. Still, I persevered and could adapt to a different way of thinking, which has shown up as a skill in nursing manifested as adaptability and understanding interactions on a molecular level further.  


How was it applied -The knowledge I gained in organic chemistry has helped broaden my understanding of pharmacological interventions during patient care. I can quantify interactions that medications have with each other and how they will affect a specific disease process when at the bedside with a patient. My knowledge of organic chemistry has made me a safer nurse through my ability to understand and think abstractly about medication interactions that could potentially help or harm a patient.

Future: The knowledge that I have used from my chemistry classes ill continue to serve me in not only further understanding of medications as I progress through my nursing career but also are a foundation to a further understanding of pathophysiology of diseases

As previously mentntioned the ASU nursing program has allllowed me to utilize intrative larning built  across the span of my educational career. Preewquisits have allowed me to make conntinued connections between all classes and helpts me to meet the   Integrative learning is an understanding and a disposition that a student builds across the curriculum and co-curriculum, from making simple connections among ideas and experiences to synthesizing and transferring learning to new, complex situations within and beyond the campus; Demonstrated through the application of knowledge, skills, and responsibilities to new and familiar settings and through addressing complex problems.

Domain 2: Person-Centered Care

My Definition: Person-centered care is putting the patient or client in the practice of your nursing process. Regardless of disease or disease processes, everyone should not be treated as a status quo. The patient's individualized needs need to be discovered by the nurse through not only therapeutic communication but also the ability of the nurse to assess subtle cues by the patient. The nurse should consider religion, family dynamics, and best practices to provide outstanding care for the patient. When the interaction with the patient is discontinued, the patient or family should feel that the nurse did all they could to provide quality, safe, and effective care for the patient.

Where I learned it: Putting the patient and their needs first was a fundamental drive for me to become a nurse. I began to develop the art of providing person-centered care in my Fundamentals class in my second semester in the ASU nursing program. Clinicals within the Evergreen Nursing Home helped me learn the basics of quality nursing care and treat each individualized resident with quality care regarding therapeutic communication.

How it Was Applied: Utilizing Therapeutic communication learned in Fundamentals of Nursing enabled me to discover the patient's heritage, health history, and likes and dislikes. This was just the beginning of patient interactions that have helped me to evolve my nursing practice to feel more comfortable with patients; I began to develop a therapeutic flow, if you will, where the conversation was easier for me to navigate as I began to give all of my attention honestly and showed my ability to demonstrate active listening to help the patient to feel valued, cared for, and most importantly that their feelings and wishes were being not only heard but acted on.

Future: My ability to display patient-centered care will grow exponentially as I develop my nursing practice. As I gain more knowledge within the field of nursing, I will not only be able to provide therapeutic communication, but I will be able to answer some of the questions that a patient has with more experience. I have found that when delivering patient-centered care, sometimes knowing the answers to the hard questions is therapeutic.

I have demonstrated this area of the Adams Outcomes by engaging in patient communication therapeutically to gain a more focused sense of who I am taking care of as a student nurse. The experiences outlined above are just a few instances in which I have successfully. I thrive in the areas of oral communication by being consistently a team player and effective therapeutic communication with patients.

Domain 3: Population Health

· My Definition: Population Health is the community in which one lives, the surrounding areas, and the health outcomes of that population. Population health can also be on a larger scale, county-wide, statewide, and globally. It is any entity, whether public health, behavioral health, or any community or government organization overseeing health care. This is evident in the American public health system.

Where I learned it- I learned population health and management was in my Rural Health class, where we studied the primary, secondary, and tertiary health prevention models on a community scale, state, nationally, and globally. We also went to our rural health clinics, where we actively participated in public health and hospice.

  How it is Applied During my clinical rotation in Creed, the population health domain could not have been more evident. One public health nurse was a full-time DPN in the town and was responsible for the entire community. She acted as EMS, the town Doctor, the town lactation consultant, and above all, the city advocate through her research in the mineral content of the whole village. She worked with CU Health to study the water content to benefit her community. I went on house calls with this nurse, witnessed her part in an emergency action response for the schools, and saw the plethora of patients who came to public health for her care.

Future-  I will take what I have learned from rural health and the nurse in Creed with me in my knowledge and what I saw in her practices as a future nurse. Watching this nurse perform a constant nursing assessment on the entire town helped to inform me of how you live and carry health outcomes with you as a nurse. Your goal is to provide utilitarian care for as many people as you can, and I choose to have this aspect in my mind at all times as a nurse to benefit who I can and when I can at every possible moment.

I have demonstrated that experiences like I had in Creed have helped me to understand that being a nurse is identifying a problem and then coming up with implementations to solve these problems, whether it be a small rural community or a nation as a whole every single person can benefit from a nurse like me who truly understands the importance of identifying and solving problems.

Domain 4: Scholarship for the Nursing Discipline

My Definition:As a nursing student, I see nursing scholarship as the foundation for  improving care, shaping policies, and educating future nurses. It involves research, applying evidence to practice, integrating knowledge, and teaching others. I must understand how to critique research, apply findings to care, and act ethically. While research generates new knowledge, evidence-based practice focuses on using that knowledge, and clinical expertise to guide decisions. This prepares me to work with healthcare teams, and importantly, to address health disparities with compassion and commitment, providing high-quality, patient-centered care throughout my nursing career.

Where I learned it-  My research course with Adams State University's nursing program gave me this experience and taught me how to critically assess studies for their applicability to nursing care.  Through my research course I stuided alarm fatigue , My nursing fundamentals class taught me to leverage research, patient preferences, and clinical knowledge to make informed decisions. However, during my clinical, I genuinely put these skills into practice with the invaluable guidance of my preceptors. Ethics lessons instilled in me the importance of conducting research ethically.

  How it is Applied  The integration of evidence-based practice (EBP) is evident in all of my clinical experiences, whether through observing nurses apply best practices or learning new techniques rooted in current evidence. Each clinical rotation offers an opportunity to enhance my skills by applying EBP principles in real-world settings. In my medical-surgical nursing class, we continuously engage with the latest research on assessments and disease processes, staying updated as new studies emerge. This consistent exposure to evolving evidence ensures that my knowledge remains current and aligns with best practices in patient care.

Future- In my future nursing career, carrying these practices forward means consistently delivering high-quality, patient-centered care grounded in the latest evidence. I’ll use evidence-based practice (EBP) to make informed clinical decisions, adapting to new research and techniques to improve patient outcomes. Whether managing complex cases, performing assessments, or educating patients, I’ll apply the most current best practices while considering individual needs and preferences. Collaborating with interdisciplinary teams, I’ll also contribute to improving healthcare policies and processes. By committing to lifelong learning and staying updated with emerging studies, I’ll ensure my practice evolves, enabling me to provide safe, effective, and compassionate care throughout my career.

Domain 5: Quality and Safety 

​

My Definition:Nurses and nursing students must be the change they wish to see within the healthcare stream. Patient care and deficits must be appropriately identified to progress and propel change within the healthcare system. Nurse managers should create a culture of safety as a top priority by ensuring a “just culture” in which nurses can voice concerns without fear of being punished for errors or near misses. Nurses should be current with current evidence-based practice and continue education regarding patient safety and OSHA standards.

Where I learned it-  Every clinical experience is a learning opportunity, especially when it comes to providing the best standards for patient safety. I've come to understand that growing and learning within the hospital is not to be feared, but rather embraced as a valuable opportunitie.  With each clinical, I've identified at-risk populations and it's my lifelong goal to ensure that every patient I encounter receives the same quality of care and support I would want for my loved ones.

  How it is Applied  The nursing domain is brought to life through the ADPIE process. The nurse is at the forefront, assessing issues within the healthcare stream, diagnosing problems, planning the proper interventions, and evaluating the outcomes. The nurse is the driving force behind the changes that need to be made and executes the change plan.

Future-  In my practice, I am committed to continually identifying and addressing gaps in patient care, practice, and safety guidelines within the healthcare system. When a gap is identified, I will follow the nursing process to implement a change. This commitment to improvement will be a cornerstone of my nursing career, as I use my professionalism to bring about positive change in my community, workplace, and life. ·

To meet Adams State University Outcome III I recognize that, as a nursing student, I must be an active part of the change I wish to see in healthcare. Domain 6 : Interprofessional Partnerships

​

· My Definition: In my opinion, this domain represents how crucial the nurse is in the promotion of strong interprofessional relationships within the care system, whether in hospitals, long-term care, rehabilitation, or public health. The nurse's role is to ensure that outcomes of interventions are effectively communicated across disciplines, ultimately creating a seamless experience for the patient. The nurse must have a team-based goal in mind to draw on expertise from each discipline and allow for proper quality and safety of care. The nurse relies on evidence-based practice and care to ensure that all decisions are made with foundational research and safety in mind, allowing the nurse to further grow interpersonal connections based on respect, trust, and a foundation that is most beneficial to the patient.

Where I learned it- Where I learned it – Through my Health Assessment class and Med Surg I and II classes, interdisciplinary roles were often taught and discussed. Whether discussing the role of physical therapy, respiratory therapy, radiology, or other professional disciplines within the hospital system, these topics were central to our learning. Having never worked in the hospital before nursing school, the concepts of interdisciplinary teams were all abstract until these foundational classes brought the scope and intricacies of these professions to reality.

  How it is Applied- I have applied this domain throughout my time at Adams in the clinical setting, particularly in observing interdisciplinary interactions. I’ve witnessed numerous exchanges, such as a nurse communicating with a doctor using the SBAR format. I have also engaged in many interprofessional conversations, watching and learning from other disciplines. For example, respiratory therapists have educated me about specific interventions and their process to assist with helping the patient with a compromised respiratory effort. At the same time, radiology has taught me about ultrasounds, how to understand them, and how to utilize them as a tool for different procedures. s, like the bubble study.

Future- In my future nursing practice, I will prioritize interprofessional collaboration by actively engaging with healthcare team members to ensure seamless, patient-centered care. I will continue using communication tools like SBAR to maintain clarity and efficiency in exchanges with doctors and other disciplines. I will also seek opportunities to learn from specialists, such as respiratory therapists, radiologists, and lab technicians, to deepen my knowledge and provide thorough, evidence-based care. By fostering a culture of interprofessional collaboration, I will contribute to the best patient outcomes, as communication and cooperation go hand in hand.

The Adams outcome of teamwork has been met by my continued engagement with the interdisapllanry team and recognition of what stratagies  work and dont work within the team dynamic.  My utilizaiotn of effective communication will help me to meat this outcome throughout my nursing career.

Domain 7: Systems-Based Practice 

​

· My Definition :Nurses promote change across all healthcare settings. The nurse must have the ability to look at the "big picture" no matter what field of health care they are in, weather, administrative, hospital setting, long term care. The particular system in which the nurse works needs to align with the nurse's personal values and vice versa. The nurse is responsible for recognizing gaps in health systems as they evolve and advocate for populations that are negatively impacted. The nurse is also responsible for helping to design systemic system based improvements.·      


Where I Learned It:  During my Nursing Management Rotations within the Adams State School of Nursing , I had the opportunity to go with an ED nurse manager to participate in the planning of a  "tabletop" Mass casualty accident( MCI); the tabletop included administrators at the public health level for the state, Salida town entities such as public health, EMS the hospital mental health professionals and administrators from within the hospital system. This highlighted the importance of the mesosystem, the more extensive state-level macro system's interaction, and the interplay between them. The table top consisted of identifying possibly vulnerable populations such as children and how that would play into a scenario like this.

  How it is Applied I applied this approach within the Adams State community by recognizing that access to lactation rooms was compromised. I brought this issue to the attention of the Adams State Title IX Equity Board, which facilitated changes that would benefit the broader macro system of public health. By utilizing the nursing process, I assessed these gaps and worked to coordinate the microsystems to implement a system-based change.

Future—Moving forward in my nursing career, I will continue to integrate systems-based practice and use the nursing process to identify changes that either need to be made or are going to be made by looking at the broader picture of the micro and macro systems of planned implementation and possible effects and outcomes that could happen down the line of the system in which I am working.  I will continue to identify gaps and  if changes are within my scope and authority will take nessicary steps to work towards them. I hope to be a driver of change for the better!

This domain has allowed me to meet my Adams outcomes by allowing me to apply throughout  my nursing education. I have integrated this domain not only in the hospital system but also in my every life as a parent, looking at the school system that my child goes to and how it is affected by local and state policies. I will carry this Adam Outcome throughout my life. 

Domain 8: Informatics and Healthcare Technologies

Where I learned it:In my healthcare informatics class, I learned how to use technology to improve patient care and manage healthcare data. However, the most important lesson I learned was the value of teamwork in healthcare. I gained experience working with electronic health records (EHRs), documenting accurately, and understanding how to protect patient privacy. . I practiced using healthcare software to enhance communication, realizing that it's not just about the technology, but about the people who use it. These skills have prepared me to use technology effectively in my future nursing practice to improve both patient care and teamwork, and I now understand that the two are inseparable

  How it is Applied I apply what I have learned in healthcare informatics by using electronic health records (EHRs) to accurately, securely, and in real-time document patient information. This ensures that all members of the healthcare team have access to the most up-to-date information, which improves communication, reduces the risk of errors, and supports continuity of care. I also utilize data from EHRs to monitor patient outcomes, identify patterns or trends, and adjust care plans as needed. For instance, I can track vital signs or lab results over time to detect early signs of complications.  These skills enable me to make well-informed decisions, provide safer care, and improve patient outcomes while working efficiently as part of a collaborative healthcare team.

 This domain allows me to meet the Adams State University outcomes by applying critical thinking to track patient data and healthcare trends over time, which is essential for delivering quality care. Using informatics as a tool, I can analyze trends to identify potential problems both at the bedside and across the healthcare system. This skill helps me piece together data, recognize patterns, and make informed decisions that improve patient outcomes. By identifying trends early, I can anticipate issues, implement evidence-based interventions, and contribute to systemic improvements in healthcare practices.

Domain 9: Professionalism

· My Definition:  The nurturing professional needs to be the pinnacle of a moral compass; they need to exist as someone out to serve all of humanity. To develop the identity of professionalism, the nurse needs to recognize their individual barriers when it comes to. emotional intellgece . The nurse needs to incorporate their history of life experiences to draw from and influence their voice. Though professional identity is not made in one day, it is cultivated throughout the nurse's profession as the nurse not only fosters confidence.

Where I learned it-I learned about developing a professional nursing identity during various courses and clinical experiences throughout nursing school. Specifically, in courses related to professionalism in nursing and emotional intelligence, I gained insight into how a nurse’s personal history and life experiences influence their practice and voice within the healthcare environment. My clinical rotations also provided real-world opportunities to observe how nurses interact with patients and colleagues, reinforcing the importance of cultivating a strong moral compass and emotional awareness. In leadership and ethics courses, I also learned how confidence and self-awareness evolve over time, helping nurses navigate challenges and build their professional identity.

  How it is Applied I have excelled in cultivating my professional identity, especially in therapeutic relationships with patients. I have found that through my life experience,  I have been humbled, and I have brought that humbleness into my practice of service and care within the nursing profession, and it has helped me to connect with patients. I also can identify gaps in care due to inequities, best practices, and where things can fall through the cracks.  Through my nursing education, I could discuss gaps with my nursing faculty,  whether a preceptor or my director of nursing. I have noticed through my education my growth in being able to lead the development of opportunities such as helping to improve the quality and access of   lactation rooms on the Adams State University campus which I was directly involved with .

Future- As I reflect on how seamlessly I have been able to step into the role of a nurse, I am excited to continue identifying gaps, collaborating professionally, and driving meaningful change. As I progress in my career, I am eager to further develop my leadership skills, particularly in identifying and addressing areas for improvement within patient care in whatever facility I find myself in throughout my career. 
As a new nurse, I didn't feel confident about stepping into the role of a leader until my senior year at Adams State University. However, through my education and clinical experiences, and with the guidance of my mentors and the support of my peers, I've gained the confidence to take on roles that can affect change for the better. I look forward to how I can drive the change I wish to see in the world, one patient at a time.

Domain 10: Personal,Professional and Leadership Development

· My Definition: The novice nurse must bring advocacy, adaptability, and boldness to the profession. These attributes help maintain the nursing profession's retention.  In addition to competency, a nurse must prioritize self-care. Through self-reflection, they can recognize triggers contributing to mental and physical exhaustion, enabling them to take proactive steps to maintain their well-being.

Where I learned it- During my Medical-Surgical II rotations, I encountered a series of life events that prompted me to seek guidance from my professors and preceptors. I wanted to learn how to reflect on and redirect negative perceptions toward a more professional and positive outlook—not only to reduce external sources of stress but also to foster the confidence I needed to step into the role of a nurse. Since then, I have been proactive in identifying the types of environments where I know I will thrive as a nurse and assessing those that may not be conducive to my health and well-being. Being empowered and confident within the work environment that I am in will allow me to step into the role of the leader.

  How it is Applied -As I approach the completion of my degree at Adams State University, I have applied this domain by continuing to evolve into a nurse who prioritizes self-care practices such as regular exercise, proper nutrition, and seeking guidance from the many mentors fostered by the Adams State School of Nursing. I have embraced leadership by cultivating strong relationships and networking with healthcare professionals. This allows me to gain insight into positive workplace environments and ensures that I am in settings that promote educational growth, and preventing stagnation in my professional development.

Future-  In the future, I plan to utilize this domain by carefully evaluating different New Grad residency programs. I am weighing options based on which programs offer the most opportunities for continuing education, as well as where I will have the greatest potential for professional growth in my new career. I am considering every aspect of the workplace, from the orientation process to the wellness benefits provided and the opportunities for ongoing education. Ultimately, I want to work in an environment in wich I align with the mission and values of the organization, where I feel professionally fulfilled, and where I can provide the best possible patient care.

Adams Outcomes Adams Outcome III Personal and Social Responsibility : Foundations for lifelong learning.

I have demonstrated this Adams Outcome and will continue to prioritize self care and emotional well being throughout my nursing practice so as to always have a foundation that will allow for lifelong learning.  By entering a New Grad Residency program I will be able to ensure that as a new nurse I will have a strong foundation for my new career within the feild of nursing.

Significant Learning Experience 

     My educational journey in the Adams State Nursing Program has been an eye-opening experience that has given me a deeper understanding of myself and what it truly means to be a nurse. Among the many noteworthy experiences I've had thus far, the one that stands out the most was during the Fall of my Junior year when our cohort participated in rural clinical experiences and worked at a local long-term care facility in Alamosa. Through these clinical rotations, I gained valuable insights into the diverse nature of the nursing profession, and I discovered my passion for caring for older adults. Every patient and resident I interacted with during my clinical experience taught me something valuable. Working with older individuals during such a transformative and vulnerable time was both an honor and a privilege. 
My rural health clinical was also a meaningful experience for me as I was part of hospice rotations and had the opportunity to work in various public health locations throughout Colorado. Each experience was unique, and I acquired new skills regarding patient-centered care and interaction.
